WHAT IS IN HERE

  change-log.xlsx Start here. Three sheets:
                                    KPIs success rate, standard and
                                                    emergency ratios, calculated
                                                    automatically
Change log one row per change, with
                                                    dropdowns for type, risk and
                                                    outcome (three example rows
                                                    included - delete them)
                                    Standard changes eight pre-approved templates
                                                    to adapt
  change-record.pdf One-page record for a single change.
cab-agenda.pdf 15-minute weekly CAB agenda and chair rules.
risk-and-approval-matrix.pdf Risk scoring questions, 3x3 matrix, who approves
                                  what, and blackout periods.
post-implementation-review.pdf Review sheet for failed, emergency and
                                  high-risk changes.
csv/ Plain CSV of both sheets, for Google Sheets or
                                  any tool that dislikes .xlsx.

HOW TO START

  1. Open change-log.xlsx, delete the three example rows, and log every change
you make for one week. No approvals yet. You are buying visibility.
  2. After a week, look at what repeats. Move those into the Standard changes
sheet so they stop needing approval.
  3. Publish the risk matrix and who approves what, then start the 15-minute CAB.
  4. Watch the KPIs sheet. Success rate above 95%, emergency ratio below 5%.

These are starting points, not standards. Rename them, cut the fields you do not
need, and keep them somewhere your team will actually look.
